Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We are currently seeking a Triage RN to working in our West Valley (Avondale) facility.

The Triage RN is responsible for reviewing cases for medical and psychiatric appropriateness as well as approval for transfer of care requests that meet medical necessity criteria. The Triage RN will work with staff stationed throughout the CBI systems and care network and remain available throughout assigned shift to ensure constant availability for calls and/or telemedicine assessment requests. This position is responsible for triaging hospital, UPC, RRC transfer requests, triaging with outreach teams for transport appropriateness, and documenting all services provided.

Skills/Requirements

Must be currently licensed as an RN, without restriction, with the Arizona Board of Nursing.
